I need to know the proper format to writing a mitigating missive to a judge for a sentacing trial? Please Help?

My boyfriend is currently individual held in incarcerate, I am 9 months pregnant and have begin pre-labor, I dont have much time to get hold of him out. I need to know what the best format to writng a communication to the judge to provide mitigating evidence. He is facing a probation sacrilege and a DUI charge, 2nd offense.
