Is there a legal reason why my ex-husband should have...?
my physical address. In the past, he has used this address to have me harrassed. He would spend money to make sure I miserable rather than pay child support. I use a PMB for mail and packages from him and his side of the family.
Is there any legal reason requiring me to provide my home address? History of domestic violence with me as the victim.

No, your husband doesn't have to know anything about you and your life. All you have to do is find a place to drop off and pick up your child and that is it. As long as you both have access to your child that is all that matters

no. a restraining order against him for the past domestic violence would help support your argument against giving him your address if you ever had to go to court on it
