How do I legally changing your name in NY state?

You can just go to your city hall and they will give you the forms. You need to have your drivers' license or passport, and some other forms of identification which prove you live where you say you live (like a utility bill). You should probably call your city hall before you go to make sure you have the proper documentation because nothing sucks more than taking the time out of your day to go to the office, stand in line, get talked down to by surly civil servants only to be rejected because you didn't bring an i.d. In my city hall, it's the same department that handles marriage licenses, so it may be the same for you. Good luck!
