It means "with the necessary change having been made."

In other words, if there is an existing agreement - say, a lease for one property at one price - and the parties want to add another property or change a term that recurs through out, then rather then going all through the lease and amending each reference they just say the lease applies to the new property "mutatis mutandis." In other words, all the terms should be read in light of the new change.

There are risks in doing this (the contract may not be clear, or the parties may disagree on what the changes should be), but that's essentially what it means.
