How should i word it?
a lady hit my car on the 1st of august, said she had insurance at the time, but really doesn't. i finally contacted her and she verbally agreed to make payments to fix my car so we dont have to go through the trouble of court. i want to get it in writing since she already lied to me once... how should i word it? i tried to find any already made form or example online but had no luck.

You sound like a nice person that probably has insurance and you are trying to do the best thing to get your damages back without hurting the other person too badly. However, the other person is not as nice as you. First, they were driving without insurance. Most states now have laws requiring insurance and even if they don't, responsible people don't drive without it. Second, she lied to you concerning her coverage, probably to change whether you would call the police or not.
There are a million differnet ways to arrange this, but if she skips out on you in the middle, you will be left holding the bag unless you then take her to court. My guess is that you will have to eventually do this anyway to get your damages back.
Therefore, I would recommend not taking a chance. If you have comprehensive coverage with uninsured motorist coverage, I would inform my insurance company regarding this accident with an uninsured motorist. They will fix your vehicle and then go after the other person for you. Otherwise, you are probably violating the terms of your insurance and they may not be able to help you as much 6 months to a year later after your car has been fixed and this person is still dragging their feet.

BTW, if there were any witnesses to this accident, be sure to firm up their contact information immediately so you will have someone to potentially testify about what really occurred.
Just write it in simple English and both of you sign it. It doesn't need to be fancy. "_____ agrees to pay ____ the amount of $____ each ____ weeks for the purpose of ______", etc.
