Am i required to list ALL of my previous employers?

I'm a 20 yr old college student and i recently quit my job due to the ongoing harassment by the store manager. I need to find a new job but on all applications they ask for my previous employers. I don't want to put down my last job because a) when it asks why i left i'm not sure if "harassment" is appropriate to write down b) i don't want to check no on the "may we contact" box and c) if they do contact him i know he'll give me a terrible reference because i got him into some serious trouble by reporting him and all the hell he put me through.

Any help would be great

How long were you at your last job? You are not required to write down your last employer if you dont want to but I am sure in your interview the person interviewing you will ask about the gap in your work history so have some excuse made up for that gap. school is ALWAYs a good one!

It really depends on where you are applying, the main thing you need to look at is rather or not the place you are applying to will do a credit check on you. If they do, then you might want to list it, and just explain the circumstances of your leaving. If they do not, then there is no reason for you to do that.
