How do i go about reporting a grocery store to the FDA or proper authorities?

To make a long story short i move to this area back in March and since then the local grocery store that i have been going to has been selling bad food, i thought it was just a one time thing with my milk going bad way before the expiration date then it happen again and then again, the same thing happens with the meats as well, at first i was just going to ignore it and buy those kind of food else where (i live in a small town so there arent that many choices) but the other day i went and got some grapes from there and before i could even get home (because like most people i ate some of the grapes in the store) i was sick to my stomach and it was coming out both ends, after i got sick my neighbor also told me the same thing happen to them but with the hot dogs from that store they went bad after two days, where would i go to inform the higher ups on what is going on?

Answers:
Call your local health department and report them - they're responsible for investigating this kind of problem.

County health department will look into it if you make a call!!
every town has a local (health department) i would start there tell them every thing and by law they are required to report them. good luck and dont buy any thing from the store until this mater is cleared up .and just keep a camera handy,it might be use-full u could take pictures of the rottin food.
Small town?
Then everyone else must be having the same experiences.
Boycott the place.
Why take the chance? Drive a few miles to a decent store. The reason they are still in business is because those who who buy from the store don't complain.
But I've a real problem with your eating grapes in the store. Not because you wouldn't pay for them but how in the hell., but with your knowing the CONDITION of some of the food you have already purchased, would you even consider EATING unwashed grapes in the first place!
you don't have any idea where those grapes were before they were put on display. None whatsoever.
Yeechh !
Call the Health Department immediately, and take your grapes to their office.
Call the local health department. They will send someone over there to kick butt.
You should report this to your local Health Department immediately, copy to the FDA, which can be found on line.
go here and file an online complaint

The Health Department usually has an area that checks out restaurants and stores and should be able to check into this for you.

I worked at a city Health Dept years ago and they had a section called Environmental Health. The inspectors checked the dairies to make sure farms were doing what they should when milking cows, along with checking the restaurants and grocery stores, especially when a complaint came in like this.
local health dept.
County or city health department handles these things. The FDA can't get involved.
