How does one change their complete name in New York legally?

I am Born in United States, Female , Single, no Children, a resident of New York.

You will also need to bring a certified birth certificate to submit with your petition. Then a judge must formally approve your name change and will receive a document stating your name change was approved. You will need copies of this to change your birth certificate, SSN, and any people who will be affected by the name change (Utilities, Creditors, etc.).
