Advance Directives for a Minor Child ...?
Does anyone have or know where I could locate a decent blank format for a Living Will and a Durable Power of Attorney for a 13 year old. I live in Vermont (USA).
***My son is having brain surgery on the 30th and I would like for him to be able to, at the very least, fill out his wishes and have them on file with the doctors before he goes under.

Wow, I am so sorry to hear that. You might be able to find one at an office supply, or I think that the hospital might have them. I do know you can also purchase them online, there is a CD for the computer that has them on it, and I think it is less than $50.00
