How does the NSA know who to spy on?

Surely the NSA is not tapping every phone conversation and internet search. They simply don't have the time and resourses to watch everyone in the US. Where do they start. I have a theory that where criminal records come up short, they send people to infiltrate mosques, schools, and community centers. Where do you think the spies are?

NSA only monitors the phone calls of known terrorists outside this country.
Most of what the NSA and how they do things is so classified I myself taking my best shot.
Determining whom to surveil is one of the most important skills of the NSA.
I've heard theories, and many hacking circles believe, that voice recognition filters are in place. When certain key words are spoken, the conversation is logged and recorded, and both parties are identified. Later, this data is checked by a human analyst and is either discarded, or the wiretapping process continues if it might lead them to something.

Actually, the US has had a hard time recruiting spies and double agents that can work with Islamic extremists. That may all be false information though to get them to let their guard down.
