Can you amend an affidavit of incompetency of trustee (and acceptance of office by successor trustee)?

If the document was Recorded in Official Records, can the LLP make an amendment stating that it was done in error? Can the person who falsified the statement, acting as a doctor, be prosecuted through the Court system. This document didn't surface until 3 months after the principal passed away and was found by the other agent who would have had to make the decision along with the other agent, make the person, who declared under penalty of perjury be liable for their decision.
