Last summer I saved up for 6 fillings...?
So, I got the composite resin fillings done--you know, the white ones, and one of them was put in too deep. I now need a $1500 root canal and cap to fix my absessed tooth. Is it the dentists' fault for jamming the filling so deep it infected my roots? I went to a different dentist who informed me I needed the root canal. Do I assume all side effects of the filling and have to pay for whatever goes wrong? Or is the dentist responsible to pay for my root canal?
The 2nd dentist told me the filling was put in too deep and that infected the root. I'm not trying to get something for free.

The second one might just want to get money out of you for services you don't need. Dentists are like lawyers; extremely very few are honest. The best thing for you to do is to go back to your first dentist and let him to correct his mistake free of charge/
Do not worry about the waiver. Negligence overrides all waivers/
Depends on whether you signed a waiver or not. A lot of doctors will have patients sign a waiver that prevents them from suing for malpractice. If you did not sign such a waiver you may take the dentist to small claims court and sue for the exspenses of the root canal and cap.

You should speak to the dentist who did the work and tell him what was found by the other dentist. It is not right that you should have to pay for this expensive work that was caused by his mistake. There is a board of dentistry too, that you should check with on this. I feel he should fix it for no cost at this point.
