How can I prevent losing child custody on grounds of mental illness?
I worry that if something happens to my spouse the state may remove my child from my care based on the past severity of my OCD. To help me stop worrying, can anyone tell me if I can prevent this: 1) if it is properly managed by medications; and/or 2) I demonstrate that I have help to care for my child? I know this might be a ridiculous thing to worry about. My spouse is fine. But I am applying for Disability Benefits now and I am worried about the future implications.

If your OCD is managed medicinally and you demonstrate that the child is cared for either by you, or your hired care giver, then you should be fine. It's usually found by the courts that the child's best interests are met by staying with a natural parent rather than being tossed into the overloaded system. You have nothing to worry about in my opinion.
Also, the State would have to have "cause" to remove the child, someone would have to report to protective/family services that the child looked abused, severely neglected, or starved. I am quite certain, by the sound of your question, you would neither abuse, neglect, nor starve your child.
most times the courts ( CPS ) are so far behind or screwed up they could care less about the kids
