You can have an attorney draw up the paperwork and have it notarized and witnessed or if you need to save money then I would go to Staples and pick up the power of attorney packet. You will have to have it witnessed and notarized but it is cheaper than hiring a lawyer. This is assuming the ill person is able and coherent enough to appoint you power or attorney. If not then you will have to get it court appointed to you.
if you need to make medical decisions for them, you need a health care proxy form. The patient has to be of sound mind to be able to legally choose a health care proxy. You can get it at any hospital
