Filing a petition in Missouri... please help!!?

I am requesting a name change. I bought all the paperwork online at legalcpu.com. However, when I took this packet up to the Circuit Court today, the clerk informed me there was no petition in this packet. I can't file without a petition.
My question is, how do I go about getting a petition? This is for a name change in Buchanan County, Missouri. I already know that I don't need a lawyer for this. How do I do it though? Where do I get one? And did I waste my money on this paperwork that WAS NOT cheap??
