Should an exotic dancer form an LLC?

It depends.

From a business aspect, it might be wise to separate personal income for business income as well as liability.

It also depends on how much money you are making. Are you alone, solo? Or, is there more than one?

If this is the route that you want to go, you might be better off forming an LLC and getting some other dancers together and form your own business.

You might want to consult an attorney. They will know what is best for your situation. Honestly, the laws are different in every city.
Why not. Anyone can as long as they are not an employee of a company I presume the dancer is a contracted laborer as in a 1099.
If she is in business for/by herself, I think it would better to be a Professional Corporation.

However, why does she need a liability shield; she might give an old guy a heart attack because of her risque dancing?
I THINK EXOTIC DANCERS ARE NASTY FILTHY PERVERTS AND THE ONLY THING SHE/HE SHOULD FORM is some decency by forming and wearing AN UNIFORM ...or something decent!!
