How to write Conscent Form?

My school is taking some children to a field trip and i have to prepare a Form in which the parents of the children have to sign as a proof that they are allowed to go to go to the field trip. please help me find a form like this which says that my child is allowed to go to the field trip or something like this.

You don't need a super looking official form. Just a half sheet of paper that states something like, "I __________ allow my son/daughter to attend the field trip..." Then just have a line for the guardian's signature. You should run the simple form by your office. Or heck, the school's office should have consent forms.
