You need to capitalize "attorney," since you are treating it as an title. But there may be a better way to state it to make it less awkward. If you were to say:

"I am meeting with Mary Smith, Attorney at Law." That is her title.

OR

"I am meeting with Defense Attorney Mary Smith."

OR

"I am meeting with the attorney, Mary Smith."

See the subtle differences? If attorney is the noun and general, you don't need to capitalize it. If it's part of the name or title, it needs the "A." That's what I think anyway.
