Is it possible to legally change your identity and not be found by anyone for the rest of your life?

The IRS will always find you. But I'm sure you could hide from your parents and ex-girlfirends pretty easily.

No. If you change your identity it is public record. However if the government does it under witness protection there is no public record. So in that case, yes.

A change of name is public record. Also plenty of places will require you to issue a public notice of your change of name, which creditors follow very closely.
